This page contains forms that are required for Admission to WINGS.  If you will not be accompanying your child on Admission day, please contact our office at 320-593-0440 to have the Admission forms sent to you, or you can print them from the list below. This list is only a partial list of forms, but are the most important ones needed for Admission.

A physical is needed for admission to WINGS, it is preferred that your child obtain one prior to admission.  Please print out the Physical and Mantoux/Free of Communicable Disease forms from the list below and provide it to you health care provider at the time of the physical.  If you cannot obtain a physical prior to admission, our staff will schedule a physical for your child at Affiliated Medical Center in Litchfield. The cost of the physical is your responsibility.
